Select Circle Health Plan
is designed to keep your employees happy as well as healthy. As the preferred
provider organization (PPO) for Cardinal Health Alliance, we've established
a diverse network of outstanding physicians, hospitals and ancillary
facilities/services to give your employees the choices they want. And
we take it a step further by allowing the freedom to seek treatment
from outside our network as well.

Quality
Care
We're affiliated with some of Indiana's finest physicians offering
a comprehensive range of medical specialties, as well as family practice,
internal medicine and pediatrics. (More than 80% of our participating
physicians are board certified in their specialty.)
Convenience
Your employees also enjoy the convenience of finding a network
physician or care facility close to home. In fact, Select Circle participating
hospitals are located throughout east central Indiana including:
- Ball Memorial Hospital – Muncie
- Blackford County Hospital – Hartford City
- Henry County Memorial Hospital – New Castle
- Marion General Hospital– Marion
- Jay County Hospital – Portland
- St. Vincent Mercy Hospital – Elwood

An added convenience: Neither you nor your employees will ever
need to handle claims paperwork whenever a Select Circle provider is involved.
Instead, Select Circle physicians and hospitals file the claims for you,
then insurance payments are made directly to that provider.
Savings
We have negotiated agreements with our network providers to ensure
quality care at substantial savings. (These agreements include hospital/facilitiy
discounts, physician fixed fee schedules and no balance billing.) As
a result, your employees enjoy considerably lower out-of-pocket expenses
–just by choosing one of the numerous healthcare providers affiliated
with the Select Circle network.
Choice
On the other hand, your employees are not restricted to Select
Circle provides. They have the freedom to seek care from any hospital or physician
they choose and still receive reasonable benefits coverage.
In cases where a family is covered by the Select Circle Health Plan,
there is still the flexibility of choice. Individual family members may
elect to use a network provider while others may seek care outside the
network.* If you need to see a specialist, you or your referring primary
care physician will have many to choose from within the network. As always,
you still have the freedom to choose a specialist from outside the network.*
* Subject to the employer's plan provisions for non-network provider
care such as higher out-of-pocket expenses.
